My hammer head flew off in the middle of a demo job

I was out at a client's farm in Oregon last Tuesday, working on fixing a old gate hinge. My 3-pound cross peen hammer head just popped right off the handle after maybe 20 swings... I had wedged it myself a month ago with a metal wedge but it still came loose. Ended up chasing the head across the gravel for 5 minutes before I caught it. Has anyone had better luck with using epoxy along with the wedge to keep it stuck?

Yeah I used to think a good wedge job was all you needed but after the same thing happened to me on a framing hammer I switched to epoxy. Never had one come loose since then.

Whoa, hold up! I gotta disagree with you there. Epoxy is great and all but it's a one-way ticket if you ever need to replace that handle (and you will, trust me). I've had wedges hold for years on hammers I beat the tar out of, just gotta put 'em in right, you know?
