Coolant concentration checked after 5 years, what a wake up call

Was having tool life go to crap on the same job for months, finally grabbed a refractometer and checked the mix. Reading was at like 2% when we thought it was 7%, bacteria was growing in the sump too. That buildup made the coolant act like water and I never noticed because the pressure stayed the same. Now I check every Monday morning before the first run, has anyone else had this sneak up on them from just trusting the auto mixer?
