Caught a flash fire on a job in Gary, Indiana, now I check gas lines twice
Was on a tank repair at the BP refinery out in Gary, Indiana, back in March. We were purging a vessel and someone skipped the 15-minute gas check before we lit off. Flash fire rolled through the bay, singed my apprentice's sleeve, nobody hurt, but it shook me. That one miss added 3 hours of safety paperwork and a company-wide retraining. Has anyone else had a near-miss that made them redo their whole pre-job routine?

That flash fire story hits close to home, not because of a fire but because of a valve. Had a job years back where a guy cracked the wrong flange on a line we thought was drained and got a face full of process water, nasty stuff. Didn't hurt him bad but it sprayed everything, ruined a control panel, and shut us down for two days. After that I started tagging every single line myself with my own tape even if the permit said it was clear. My foreman thought I was being slow until the next crew had the same thing happen on a different unit. Now half the guys on my shift carry their own tags, so I guess the paranoia spread.
